Change is a fixed point in time. There is a clear before and after, like the day you move house. Transition is the process of coming to terms with and getting used to the change. It often starts before the change (finding a new home, packing boxes), and can carry on for a long time after the change has taken place. Transition can feel bumpy, even when the change is something good.
